Our incident reporting and fact-finding processes ensure that effective reporting, investigation, follow-up, and analysis of incidents are included in our operations.

The company cultivates an open atmosphere that encourages prompt, honest, and accurate reporting of all employees’ and contractors’ incidents. Human Performance Improvement teaches us that we are all fallible and make mistakes. Learning from the past is fundamental to help us create a safer future and promote continuous improvement. As a part of our Best Defense, we require timely reporting, investigation, analysis, and follow-up on incidents. We utilize world-class tools and methods to ensure that we learn from our mistakes. Examples of these tools and techniques include:

Depending on the severity matrix or serious injury and fatality potential, a learning team will be developed at the site or in coordination with Corporate Safety or Operations Excellence Team. Learning teams are led by trained Vistra leaders who will facilitate discussions with the learning team. Lessons learned from the team are shared with other locations on the Weekly Safety Learning Call to improve safety and health across the company.

Vistra documents its incidents into an incident management database, Archer GRC. The identified incident findings and learnings are entered as a condition report into the database by the site-level Corrective Action Program Coordinator. Corrective actions must be completed and verified before the issue is considered resolved.
