Vistra Safety Bulletin – August 2023

  • Over the past two years Luminant has experienced over 100 PSIF incidents (including injuries and near miss events), with over half of those incidents involving Business Partners (Contractors)
  • Further review of the incidents identified 3 existing areas of our safety program that can ensure we have defenses in place to minimize the potential for serious injury
    1. Understand the Contractor Coordinator Process and Know Your Role as a Contract Coordinator
  • All Contract work being performed at Luminant plants must have a designated Contract Coordinator
  • The Contract Coordinator must review and approve all work being performed on the plant site
  • The Contract Coordinator Checklist must be reviewed and signed by both the Contract Coordinator and Business Partner Representative
  • For Operating Plants, Construction Projects, and/or Demolition work, all Business Partners are to conduct a Pre-Job Brief (PJB) and/or a Job Hazard Assessment (JHA) BEFORE starting each job
  • For Kosse Mine, make sure all Business Partners have a Standard Operating Procedure (SOP), Job Safety Analysis (JSA), and/or Job Hazard Assessment (JHA) for the work that will be performed
    2. Pre-Job Briefs (PJBs)
  • PJBs must be performed prior to all work scopes and activities
  • PJBs should involve all parties associated with the work scope (Contractors, Operations, I&C, Maintenance, etc)
  • Clearly identify stop criteria with those performing the work
  • Have a robust dialogue regarding STCKY activities or tasks
  • Utilize the PJB Form as a memory jogger to help facilitate the conversation
   
    3. Temporary Alteration Process
  • TAPs involving critical safety systems will require Plant Manager approval
  • All TAPs are to be documented (even if completed during shift)
  • Contract work that involves integration with Plant Operations must be reviewed and approved by Plant Leadership prior to work being performed
